Trillium Health is a community-based health center providing numerous services, including HIV/AIDS care, substance abuse/addiction counseling, behavioral health services, peer support, women's services, primary care, comprehensive support, outreach, laboratory services, harm reductions services, and nutritional counseling. They also have an onsite pharmacy.
Trillium was formed in 2010 out of the merger of AIDS Community Health Center and AIDS Rochester. The MOCHA Center joined their family in 2015. They are widely recognized as a LGBT Healthcare Equality.
Their offices are located in the Monroe Square building, near downtown and on a major bus line. They have additional locations in Bath (Southern Tier) and Geneva. Trillium's Health Outreach Project with the United Way is headquartered at 39 Delevan Street in Rochester.
