Bathrooms at Barnes & Noble are usually good, especially the Webster location.

Ellison Park's bathrooms are great.

The bathrooms at the Greater Rochester International Airport are clean and well-maintained, though perhaps a bit of a trip if you're in immediate need of a restroom.

The ladies room at the Webster Lowe's is one of the best I've encountered.
-
The Sully Branch Library on Webster Ave at the Mayor Thomas P. Ryan, Jr. Community Center and Library.

Starbucks bathrooms rarely fail to disappoint.

At the corner of Andrew Langston Way and Elm Street, within the building that serves Midtown Parking Garage, there are some nice public restrooms that are seldom used and not well known. Very good place to use the restroom downtown, where public bathrooms are not in abundance. Unfortunately, they are locked on weekends.
